New Patient Registrations
Looking for a doctor?Please contact the practice and arrange to complete a new patient registration form and health questionnaire. You will need proof of identity and proof of address with you when you register.
Please try to keep your appointment and arrive at the surgery in good time. If you are unable to make your appointment, please tell us as soon as possible so we can offer your appointment to another patient.
Out of HoursIf you need to see a doctor outside normal surgery hours, please telephone the normal practice number – 01274 224 888 – and you will be directed to our emergency on-call doctor service. Alternatively, you can call 111 for any health queries.
You can order your repeat prescription via this website by following the link above and logging in to our online services. You also have the option of deciding where you wish to collect your prescription from - either from the practice or one of the pharmacies in the local area.
Please Note: If you wish your prescription to be collected from one of the pharmacies you can either contact the pharmacy concerned to register with their collection service or let us know when you order which pharmacy to want your prescription to be sent to.
Alternatively, you can request repeat medication by indicating the items you require on the tear-off slip of your prescription, and posting it to us or putting it through our letter box.

Please allow two working days for your medication request to be processed.
On occasion a GP may prescribe you what is known as an 'acute' medication. This is a medication which has not been added to your list of 'Repeat' items, which may be because the GP wants to trial the medication to see what effect it has, or if it is something which is only meant to be used for a short period of time. If you feel that you need a further supply of these medications, please contact the surgery over the phone or via the reception desk, or use the online prescription request form or SystmOnline. In these cases, the request for medication must go to a doctor for review.
In some cases they may decide that they need to see you before they can issue a further supply of the medication. After you have made your request, please contact the surgery after 5pm the same day to see whether the doctor is happy to issue the medication, or if they need to see you first.
Note that if you have never had a medication from us before and it is not a repeat item, you will need to see a doctor before it will be issued.
There are several ways that you can get help with your prescription costs, depending on your circumstances.
Prescription Prepayment Certificates
Prescription prepayment certificates (PPC) are available in England and they can save you money.
From 1 April 2019, the charges are:
If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is £9.15, whereas a three month PPC will cost you £29.65 and a 12 month PPC £105.90.

How to apply for a Medical exemption certificate
To apply for a Medical exemption certificate ask your doctor for an FP92A form. Your GP, hospital or service doctor will sign the form to confirm that your statement is correct. At your GP's discretion, a member of the practice who has access to your medical records can also sign the form.
Your certificate will be valid from one month before the date that the NHS Business Authrority receives the application form.
The MedEx lasts for five years and then needs to be renewed. You may receive a reminder that your certificate needs to be renewed. If you don't receive a reminder, it is your responsibility to ensure that it is renewed.
